The below form below meets the requirements of TEC §22A.055. The affidavit may be completed as a notarized affidavit or unsworn declaration (non-notarized). Educational entities may determine which version to use. Additionally, educational entities may rekey the form to integrate with their own hiring systems.

An individual who fails to disclose information in the affidavit commits a Class B misdemeanor. Additionally, SBEC may revoke the certificate of an administrator who employs a person or accepted services from a service provider if the administrator was aware that the person knowingly failed to disclose information in the affidavit.
